Transition Specialist (LVN/LPN)
Health Net is hiring a Transition Specialist for a fully remote role based in California for licensed LVNs or LPNs. The position focuses on managing patient transitions and care coordination.

The Transition Specialist will facilitate the movement of patients between care settings to ensure continuity of care. This is a full-time role offering a competitive hourly range between $27.02 and $48.55.
